TRESIBA LONG ACTING INSULIN

Before you READ this post, note that I only share information out of my own personal experience and that you ALWAYS need to speak to your doctor or endo before changing insulin or insulin doses.
However, since I was taught by the best medical health team and since both my kids are on Tresiba I do both of their Tresiba adjustments on my own BUT I do keep a record of my ratios and formulas.
So last year I had a Tresiba Rep visiting me at my home explaining to me how Tresiba works. I know not many health professionals don't like Tresiba and not many medical aids cover this in full, but I think its the best thing ever.
Therefore PLEASE NOTE THAT THE CONTENT IS NOT INTENDED TO REPLACE THE ADVICE YOUR DOCTOR OR NURSE GAVE YOU.
Tresiba is an ultra-long-acting basal insulin analog with a duration of action over 42 hours used to lower blood glucose.
Over time, you need to do changes to your Tresiba once every week or every second week. Since I can't phone the doctor every week do the changes on both kid's Tresiba, he does, however, get the readings from the Dexcom Clarity app.
I have a "sum" I do once a week on both kids to ensure that their Tresiba ratio is correct. I have attached a picture with the formula for those that do use Tresiba. It is also IMPORTANT to remember that Tresiba Ratio can only be changed once a week, in extreme cases formula can be changed with your doctor every 3 days. (But I only do this when I have spoken to the doctor)
FIRST, work out your AVERAGE BLOOD SUGAR by adding your DAY 1 and DAY 2 PRE-BREAKFAST MEASUREMENTS from the previous two days = Total blood sugar divided by 2 = YOUR AVERAGE BLOOD SUGAR.
THEN look where your AVERAGE BLOOD SUGAR IS ON THE CHART (KEEP YOUR TARGET RANE IN MIND) If it is below target Reduce dose - 2 units if its ABOVE Target Up dose +2 units.
BUT I use the TRESIBA 100U/ML REFILLS with Novo Nordisk 0.5 unit pen.
Out of personal experience, I do the Tresiba adjustments with 1 - 1.5 units. I have found that 0.5 makes a huge difference.
Also, keep in mind that when you switch over from one long-acting insulin to Tresiba that you need to buckle up for one Hi rollercoaster ride. This is no easy switch but ht smooth pattern at the end is so much more rewarding (although the Pancreas can still give you whiplash any time of the day)!
